BMI Explained: What It Measures and Its Limits

Body Mass Index is the most quoted number in health, printed on charts in every doctor's office and built into countless apps. It's also one of the most misunderstood. BMI is a quick, cheap estimate of whether your weight is typical for your height — useful for spotting trends across large groups, but blunt when applied to a single person. Knowing exactly what it does and doesn't capture lets you read it sensibly.

What BMI actually is

BMI is a single number derived from just two inputs: your weight and your height. It was devised in the 1830s by Belgian statistician Adolphe Quetelet as a tool for describing populations, not for diagnosing individuals — a distinction that explains most of its strengths and nearly all of its weaknesses. The metric formula is straightforward:

The standard categories

For adults, the World Health Organization classifies BMI into four broad bands. These cut-offs are the same regardless of age or sex, which is part of why they're convenient and part of why they're imperfect.

The boundaries are not magic thresholds. Someone at 24.8 and someone at 25.2 are, for practical purposes, in the same shape — the categories are conveniences for sorting groups, not switches that flip your health at a precise weight.

A worked example

Suppose you weigh 70 kg and stand 1.75 m tall. Square the height first: 1.75 × 1.75 = 3.0625. Then divide: 70 ÷ 3.0625 = 22.9. A BMI of 22.9 sits comfortably in the normal-weight band. What BMI is genuinely good for

BMI's real strength is that it needs nothing more than a scale and a tape measure — no calipers, no scans, no lab work. That makes it superb for population-level screening: public health bodies can track obesity trends across millions of people cheaply, and clinicians can use it as a fast first filter to decide who might benefit from a deeper assessment. At scale, where individual quirks average out, BMI correlates reasonably well with body fat and with the risk of conditions like type 2 diabetes and heart disease. For the job it was built for — describing groups — it works.

Where BMI falls short

The trouble starts when a population tool is used to judge one person. BMI's blind spots are real and well documented:

  • It can't tell muscle from fat. Weight is weight to BMI. A lean, muscular athlete can be flagged as "overweight" or even "obese" despite carrying very little fat, because dense muscle adds mass without adding risk.
  • It ignores where fat sits. Two people with identical BMIs can have very different health profiles depending on whether fat is stored around the organs (visceral) or just under the skin (subcutaneous). Visceral fat is far more strongly linked to metabolic disease, and BMI is blind to it.
  • It varies by age, sex and ethnicity. Older adults tend to lose muscle and gain fat at the same BMI, women naturally carry more body fat than men at any given value, and some ethnic groups face elevated health risk at lower BMI thresholds than the standard cut-offs assume.
  • It doesn't apply the same way to everyone. For children and teenagers, BMI is interpreted against age- and sex-specific percentile charts rather than the adult bands. During pregnancy the adult categories don't apply at all, since healthy weight gain is expected.

None of this means BMI is useless — it means a single value should never be read as a verdict on an individual's health.

Better and complementary measures

Because BMI says nothing about body composition or fat distribution, pairing it with one or two other measures gives a much truer picture:

  • Waist circumference directly captures abdominal fat, the type most tied to health risk, and adds context BMI can't. Combined with hip measurement it feeds the waist-to-hip ratio.
